• Pericardial cysts are rare with an estimated incidence of 1:100,000. • The most common etiology is congenital. • Management may range from conservative observation to invasive intervention. • Literature review identified 14 cases of giant pericardial cysts in the past 10 years. • Multimodality imaging is crucial for diagnosis and management.
