Health-Related Quality of Life of Patients Undergoing Haemodialysis: A Structural Equation Modelling Approach

Abstract

AIM: To develop and test a hypothetical model of the relationships between the health-related quality of life of patients undergoing haemodialysis and associating factors including autonomy support from healthcare professionals, basic psychological needs, motivation, resilience, self-management. DESIGN: A cross-sectional survey design. METHODS: Data were collected using a self-report questionnaire at four haemodialysis units, including one hospital and three clinics in Seoul and Gyeonggi-do, Korea, from March 2 to August 5, 2022. A total of 232 adults participated in this study, with data from 208 participants included in the analysis. The hypothetical model was examined using structural equation modelling. RESULTS: Autonomy support from healthcare professionals, basic psychological needs, motivation, self-management, and resilience accounted for 50.4% of the variance in physical quality of life, while autonomy support from healthcare professionals, basic psychological needs, and resilience accounted for 39.2% of the variance in mental quality of life. Basic psychological needs were significantly associated with physical quality of life and mediated the relationship between autonomy support from healthcare professionals and physical quality of life. Basic psychological needs and resilience were significantly associated with mental quality of life. Moreover, basic psychological needs mediated the relationship between autonomy support from healthcare professionals and mental quality of life. CONCLUSIONS: Autonomy support from healthcare professionals and basic psychological needs, as well as patients' resilience, significantly impact the health-related quality of life of patients undergoing haemodialysis. IMPLICATIONS FOR THE PROFESSION AND/OR PATIENT CARE: Interventional programs should be developed that promote autonomy support from health care professionals and enhance patients' basic psychological needs and resilience. REPORTING METHOD: Reported in adherence to the STROBE Statement for cross-sectional studies. PATIENT OR PUBLIC CONTRIBUTION: A total of 208 patients undergoing haemodialysis at a university hospital and three artificial kidney clinics participated in this study, and their responses were used in data analysis.
