Kommerell Diverticulum in a Right-Sided Aortic Arch

Abstract

Kommerell diverticulum is a rare aortic anomaly described by an aneurysmal portion of the descending aorta at the origin of an aberrant subclavian artery. We report an interesting case of a 57-year-old man who was found to have an aberrant origin of the left subclavian artery from a right-sided aortic arch arising from a large aneurysm in the proximal descending aorta. We describe our surgical strategy to resect the Kommerell diverticulum and to repair the proximal descending aorta under deep hypothermic circulatory arrest through a left posterolateral thoracotomy.
