Abstract
To date, no cases have been documented in the literature describing the use of core decompression combined with allogeneic fibular grafting for the treatment of refractory transient osteoporosis of the hip (TOH). Case presentation: This case study reports a patient with refractory TOH who underwent core decompression combined with allogeneic fibular grafting. At the 105-day and 472-day follow-ups, hip MRI scans showed normal findings, and the patient reported no weight-bearing pain, indicating a successful treatment outcome. Core decompression is known to shorten symptoms of TOH, while allogeneic fibular grafting can strengthen the femoral neck. The combination of these two approaches not only promotes rapid recovery from TOH but also reduces the risk of femoral neck fracture, thus representing a successful treatment strategy.
