Abstract
INTRODUCTION AND IMPORTANCE: Ectopic adrenal tissue is a rare entity, particularly in the ovary. Its identification is important to avoid misdiagnosis with more clinically significant tumors. CASE PRESENTATION: We report an incidental finding of ectopic adrenal cortical tissue within the ovary of a 54-year-old postmenopausal woman undergoing hysterectomy for endometrial cancer. A golden-yellow nodule was observed in the left ovary, with histology consistent with adrenal cortical tissue. CLINICAL DISCUSSION: Ectopic adrenal tissue is most commonly found in pediatric male patients in the spermatic cord. Ovarian involvement is extremely rare. Histologically, it can resemble steroid cell tumors or metastatic clear cell carcinoma. Recognizing this entity avoids unnecessary diagnostic confusion or overtreatment. CONCLUSION: Awareness of ectopic adrenal rests in the ovary is essential for accurate diagnosis and appropriate management.
