A comparative study of arthroscopic treatment of popliteal cyst with internal drainage and internal drainage combined with cyst wall resection

Abstract

PURPOSE: To compare the clinical outcomes of arthroscopic internal drainage (AID) and arthroscopic internal drainage combined cyst wall resection(AID + CWR) in treatment of the popliteal cysts. METHODS: In this prospective randomized study, 60 patients (October 2022) at Beijing Tsinghua Changgung Hospital were divided into Group A (AID, n = 30) and Group B (AID + CWR, n = 30). Operative duration, blood loss, intra-articular pathologies, and complications were recorded. Outcomes included Visual analog scale (VAS) score, Rauschning-Lindgren (R-L) grade, Lysholm score, and cyst volume reduction on magnetic resonance imaging (MRI). RESULTS: With mean follow-up of 12.2 ± 1.2 months, baseline characteristics were comparable (P > 0.05). Group B had longer operative time and greater blood loss (both P < 0.05). Final outcomes showed equivalent functional scores (Lysholm, R-L; P > 0.05), but Group B achieved significantly higher complete cyst resolution rates (Group A 46.4% vs. Group B 74.1%, P = 0.032) on MRI. Complication rates (e.g., ecchymosis: Group A 6.7% vs. Group B 20%, P = 0.255) did not differ significantly. CONCLUSION: Although requiring slightly longer operative time and greater blood loss, cyst wall resection with drainage provides superior cyst resolution, making it preferable for patients seeking optimal results or those with cyst inflammation, loose bodies, or multi-compartment cysts.
