Accuracy of dynamic navigation for immediate and delayed anterior dental implant placement: a retrospective study

Abstract

OBJECTIVE: This retrospective study aimed to compare the accuracy of dynamic navigation system for immediate and delayed implantation in the anterior area. MATERIALS AND METHODS: Medical records from patients that underwent implant surgery by use of dynamic navigation between August 2019 and June 2024 at the First Clinical Division of Peking University School and Hospital of Stomatology were retrospectively reviewed. Based on the inclusion and exclusion criteria, 62 patients with 102 implants (delayed group: 53, immediate group: 49) were included in the study. Implant placement accuracy was evaluated by comparing the superposition of the planned implant position and the actual implant position. Three-dimensional (3D) entry deviation, 3D apex deviation and angular deviation were analyzed. Two-dimensional (2D) horizontal deviation of the entry point and apex point, and the deviation of entry and apex point depth were the secondary parameters. The difference between delayed group and immediate group in the accuracy was analyzed by Student's t test and Mann-Whitney U test. For statistical analysis, level of significance was set to P < 0.05. RESULTS: The 3D entry deviation, 3D apex deviation and angular deviation of all analyzed implants were (1.158 ± 0.473) mm, (1.282 ± 0.528) mm, 2.947° ± 1.509°, respectively, and in the delayed implant group these values were (1.166 ± 0.493) mm, (1.283 ± 0.528) mm and 2.927° ± 1.478° as compared to (1.157 ± 0.466) mm, (1.280 ± 0.534) mm, 2.968°± 1.556° in the immediate group. No significant differences in accuracy were observed when comparing the two groups. CONCLUSION: Both immediate and delayed implantation in the anterior region exhibit similar levels of accuracy under dynamic navigation system that meets the clinical requirements. Dynamic navigation is a promising tool for immediate implantation in the anterior area. CLINICAL RELEVANCE: The accuracy analysis for dynamic navigation system of the present study may assist dentists, especially novice clinicians when performing immediate anterior teeth implantation to obtain ideal implant position. Experienced surgeon can achieve clinically adequate implant positioning accuracy in both delayed and the technically more challenging immediate cases.
