Paramedian open approach for gluteal biopolymers removal: a case series of 35 patients

Abstract

OBJECTIVES: Various techniques have been described for biopolymer removal in the gluteal region. However, the literature lacks clear guidelines on the degree of gluteal involvement. This article aims to present a safe and reproducible open approach for biopolymer removal in patients with mild to moderate gluteal compromise. METHODS: This case series includes 35 patients treated between January and March 2023 using a paramedian open approach for biopolymer removal in the gluteal region. All patients were classified as having mild to moderate involvement. The primary complications were analyzed to assess the safety and reproducibility of the procedure. RESULTS: The majority of patients were classified as grade III (94.28 %). The most common complication was seroma (40 %), followed by wound dehiscence and infection. No cases of flap necrosis or hematoma were reported. The mean time for seroma formation was 31 days. A complication-free procedure was observed in 42.85 % of patients. CONCLUSIONS: This technique represents a safe alternative for the removal of biopolymers via an open approach in the gluteal area for patients with mild to moderate involvement. Although it is not a complication-free procedure, it yields favorable aesthetic outcomes, and the reported complications were effectively managed without requiring further interventions.
