Abstract

Hysterosalpingography (HSG) remains a cornerstone investigation in the assessment of female infertility. Although generally safe, rare complications such as venous intravasation may occur and can result in diagnostic misinterpretation or systemic adverse events. We present a case of level 3 venous intravasation during hysterosalpingography (HSG) in a 26-year-old woman evaluated for primary infertility. Fluoroscopic imaging demonstrated rapid opacification of the parauterine and gonadal venous systems. The procedure was promptly terminated, and the patient remained asymptomatic. This case highlights the characteristic imaging features, clinical relevance, and preventive strategies associated with venous intravasation, underscoring the importance of real-time recognition to ensure diagnostic accuracy and patient safety.
