Abstract
PURPOSE: IC-8™ intraocular lens (Bausch + Lomb, Bridgewater, USA) is a hydrophobic monofocal lens featuring an annular diaphragm design that may reduce photophobia-a common symptom in patients with oculocutaneous albinism (OCA). We report the implantation of the IC-8™ lens in an OCA patient undergoing cataract surgery to improve visual acuity and reduce photophobia. OBSERVATION: An 80-year-old female with OCA presented with progressive visual decline and severe photophobia. Corrected and uncorrected distance visual acuity (CDVA/UDVA) was 1.0 logMAR in both eyes; corrected near visual acuity (CNVA) was 0.60 logMAR in both eyes. Clinical findings included pendular nystagmus, extensive iris atrophy, foveal hypoplasia, and newly developed bilateral cortical cataracts. Best previously documented visual acuity was 0.50 logMAR in both eyes. Standing significant photophobia, bilateral phacoemulsification with IC-8™ IOL implantation was performed. Surgery and recovery were uneventful. Six months postoperatively, CDVA improved to 0.50 logMAR and UDVA to 0.70 logMAR in each eye; CNVA improved to 0.40 logMAR in each eye. Subjective reduction in photophobia was measured using 12-Item Utah Photophobia Symptom Impact Scale (UPSIS-12), score improving from 12 preoperatively to 2 postoperatively. CONCLUSIONS AND IMPORTANCE: The IC-8™ intraocular lens may be considered as an off-label surgical option in OCA patients undergoing cataract surgery, offering not only visual rehabilitation but also photophobia consistent reduction.
