Abstract
PURPOSE: To describe a simple yet effective method to access the lens in cases of severe ectopia lentis, in which over 50 % of the lens is covered by the dilated pupil. The lens-bag complex is hooked by a chopper peripheral to and just anterior to the equatorial plane and pulled toward the center. The lens-bag complex is centered by the chopper, which allows a continuous curvilinear capsulorhexis to be executed. OBSERVATIONS: This technique enables access to the lens in cases of extreme subluxation with preservation of the lens-bag complex for intraocular lens placement. CONCLUSIONS AND IMPORTANCE: The Chopper-Capsular Fornix Hook Technique can be used to access and maintain the capsular bag by hooking and centering the lens-bag complex in cases of extreme lens subluxation.
