Combined ipsilateral fracture of the tibial shaft, lateral malleolus, and talar neck: A case report

Abstract

BACKGROUND: Talar fractures, accounting for approximately 2% of all lower extremity injuries and 5%-7% of foot injuries, are common and typically involve the talar head and neck. Avascular necrosis of the talus is one of the most challenging complications encountered in post-traumatic hindfoot reconstruction. CASE PRESENTATION: A 21-year-old man presented to the emergency department with an open wound, deformity, and multiple traumatic injuries to the right lower limb following a motorcycle accident. Examination of the right lower extremity revealed a 3-cm open wound on the distal third of the lateral cruris, deformity upon inspection, normal vascular status in all extremities, and hypoesthesia on the dorsal aspect of the left foot. CONCLUSIONS: This study emphasizes that talar fractures can occur along with tibial and fibular shaft fractures in high-energy trauma cases. However, owing to their association with complex injuries, talar fractures may be overlooked during initial assessments.
