Abstract
Posterior urethral hemangioma is a rare benign vascular malformation causing painless hematospermia. Due to its occult appearance in the non-erect state, conventional imaging often misses the diagnosis. The therapeutic challenge lies in complete excision while preserving sphincter and sexual function. We report an adolescent male admitted for 6 hematospermia. Cystourethroscopy revealed two mass-like lesions adjacent to the verumontanum, diagnosed as posterior urethral hemangioma. We performed precise vaporization using transurethral thulium fiber laser (TFL). This technique achieved complete lesion clearance and preserved function, resulting in a successful outcome.
