The Importance of Blood Inflammatory Parameters in the Assessment of Hemodynamically Significant Coronary Artery Stenosis

Abstract

Coronary artery disease (CAD) remains a major global cause of mortality, with inflammation playing a pivotal role in its pathogenesis. This study aimed to assess inflammatory ratios from blood count in CAD patients with moderate coronary lesions. This study was done at Mazandaran University of Medical Science (2013-2020) including patients with moderate coronary lesions based on fractional flow reserve (FFR) measurements. Demographics, laboratory features, neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and systemic immune-inflammation index (SII) were assessed based on FFR categories. A total of 97 patients (mean age: 62.3 ± 9.6 years) were analyzed. FFR ≤ 0.80 correlated with higher NLR (P = 0.009) and PLR (P = 0.0948). SII showed similar trends without significance (P = 0.1081). Each one-unit increase in NLR and SII correlated with a 0.05 reduction in FFR (P = 0.003); for PLR, it was 0.06 (P = 0.011). This study highlights potential links between blood inflammatory ratios and FFR in CAD patients with moderate coronary lesions. While NLR showed significance with FFR ≤ 0.80, further research is needed to ascertain their prognostic value in guiding decisions for patients with intermediate stenosis.
