Analysis of Functional and Radiological Outcome of Tibial Plateau Fractures (Schatzker Type V and VI) Managed with Hybrid External Fixator-A Prospective Observational Study

Abstract

INTRODUCTION: Schatzker type V and VI tibial plateau fractures are complex injuries with high soft tissue complications. Conventional internal fixation poses risks of infection and wound dehiscence, especially in cases with compromised soft tissue. Hybrid external fixation provides stable fixation while minimizing tissue damage, allowing early mobilization and reducing postoperative complications. AIMS AND OBJECTIVES: To evaluate functional and radiological outcomes, intraoperative parameters (e.g., surgical duration, blood loss), and postoperative complications. MATERIALS AND METHODS: This prospective observational study was conducted at SRM Medical College Hospital and Research Centre from June 2023 to May 2025. A total of 30 patients aged 18-60 years with Schatzker type V and VI fractures were included. Patients with neurovascular injuries, previous knee pathology, or ipsilateral lower limb fractures were excluded. Ethical clearance was obtained. Data were analyzed using SPSS version 24, with P < 0.05 considered significant. RESULTS: The study included 30 patients with tibial plateau fractures, with 46.7% aged 20-40 years and 73.3% being male. The right side was affected in 63.3% of cases, with road traffic accidents as the leading cause (56.7%). Schatzker type VI fractures accounted for 63.3% of cases, with a mean hospital stay of 10 days and an average surgery duration of 49 minutes. Fractures united in 12.4 weeks, with a mean intraoperative blood loss of 74.2 ml. Functional and radiological outcomes showed significant improvement, with the Rasmussen functional score increasing from 12.1 to 27.1 (P < 0.001) and the radiological score from 9.8 to 17.5 (P < 0.001). CONCLUSION: Hybrid fixation is effective for Schatzker V/VI fractures, supporting its use in soft tissue compromise cases.
