Abstract
BACKGROUND AND AIMS: Primary obesity surgery endoluminal (POSE) is an incisionless endoscopic weight loss procedure. It involves placing full-thickness plications primarily in the fundus of the stomach. Different modifications have emerged since then, such as POSE 2.0, where plications are placed in the gastric body instead. Mega-POSE plicates the gastric body by using a double-helical grasper in order to plicate both the posterior and anterior walls of the stomach. The video illustrates the steps of the mega-POSE modification for treating obesity. The procedure aims to restrict the stomach by narrowing the anteroposterior diameter and shortening the vertical length. It also aims to reduce the number of plications needed, making the procedure safer, more efficient and economical, and environmentally friendly compared with previous techniques. METHODS: The video demonstrates the use of a double-helical plication approach to achieve full-thickness plication of the stomach. Steps to achieve such plication are shown in our video. RESULTS: Mega-POSE showed great outcomes in weight loss with fewer plications compared with the traditional POSE. CONCLUSIONS: Mega-POSE is a novel modification for the gastric remodeling procedure POSE 2.0, leading to significant weight loss while being more efficient and reproducible.
