Abstract
BACKGROUND: The dorsum sellae sinus is a large transverse venous channel on the dorsum sellae, communicating with the bilateral cavernous sinuses and inferior petrosal sinuses. However, dural arteriovenous fistulas (DAVFs) of the dorsum sellae sinus have been rarely reported. OBSERVATIONS: A 55-year-old male presented with conjunctival edema and a foreign body sensation in the left eye. Through cerebral angiography, the patient was diagnosed with DAVF of the dorsum sellae sinus. The DAVF was successfully treated by embolizing the dorsum sellae sinus with Onyx via the inferior petrosal sinus approach, and the cavernous sinus was completely preserved simultaneously. LESSONS: Precise embolization of the dorsum sellae sinus via a unilateral inferior petrosal sinus approach can be used to successfully treat dorsum sellae sinus DAVFs. https://thejns.org/doi/10.3171/CASE25503.
