Abstract
BACKGROUND: Descemet stripping automated endothelial keratoplasty (DSAEK) is a commonly performed posterior lamellar procedure that is used in conditions with endothelial damage.[1] A frequently encountered complication during and after the surgery is a decentered DSAEK lenticule. There are various techniques that have been described in order to center this decentered lenticule.[2,3]. PURPOSE: The authors here demonstrate the use of an 'angled cobratip' DSAEK lenticule repositioner. SYNOPSIS: The video demonstrates the steps of making an "Angled Cobratip" DSAEK lenticule repositioner, following which the authors exhibit the use of the instrument in a decentered lenticule repositioning. The video also discusses in brief other ways of repositioning a lenticule and their advantages and disadvantages over the "Angled Cobratip." The authors have not encountered any difficulty in engaging or disengaging the needle from the lenticule during its usage. The authors have also applied for a patent for this novel design, application number being IN465916-001. HIGHLIGHTS: The "Angled Cobratip" lenticule repositioner can be successfully used for repositioning a decentered DSAEK lenticule. VIDEO LINK: https://youtu.be/fZ0QaCKSjqE.
