All-Arthroscopic Dynamic Superior Capsule Reconstruction for Massive Irreparable Rotator Cuff Tears

Abstract

Massive irreparable rotator cuff tears are a common type of rotator cuff tear and a challenging problem to handle. Superior capsule reconstruction is an effective method for targeting massive, irreparable rotator cuff tears, and an all-arthroscopic technique for dynamic superior capsule reconstruction is described. This technique uses a double layer of autologous fascia lata combined with the graft and the anchor of the graft, which is attached to the supraspinatus (SSP) stump on the glenoid side, the anchor on the humeral head side, and the anterior end of the SSP muscle to the back of the graft. This method not only has significant advantages in the thickness of the graft and the strength of the reconstructed joint capsule but also connects the graft to the residual end of the SSP muscle, playing a bridging role and fully utilizing the residual end. Therefore, this technology has the advantage of compressing the humeral head and also bridges with the residual end of the rotator cuff, restoring dynamic strength to rotator cuff function, and is more conducive to graft healing.
