Abstract
In children with complete unilateral main bronchial obstruction from foreign body aspiration, positive pressure ventilation may overdistend the contralateral ventilated lung, leading to barotrauma and tension pneumothorax. Sudden hypoxemia and hypotension should prompt rapid clinical assessment, including lung ultrasound, and immediate decompression.
